In order for Boomtown Bossier to release information regarding your tracked play you must complete and submit a signed W-2G/Win-Loss Request Form.

Once the Request Form is completed you can submit the form for processing in one of three ways:

1. By mail to the address listed on the form
2. By facsimile to number listed on form
3. In-person
